Start with ownership, not age alone
Separate balances by payer, age bucket, last documented activity and required next step. An old account with a clear appeal in progress is different from one with repeated notes and no change in status.
Look for operational warning signs
- No recent payer contact or documented next action.
- Requests for information that remain unresolved.
- Appeal or timely-filing deadlines that are not visible.
- Repeated notes that do not move the account forward.
- No named owner for the next permitted action.
Define what “worked” means
A touch is not automatically meaningful follow-up. Useful documentation should show what was learned, what remains, who owns it and when it should be revisited.
